Reimbursements
To get reimbursed for PTA approved expenses, please fill out the disbursement form and follow the guidlines below:
1) Please turn in form no more than 30 days after the date of purchase.
2) Attach your original receipt to the form.
3) The form must be signed by your chairperson before being submitted to the treasurer. Please give the completed form to your chairperson and they will review it, sign it, and submit it to the treasurer.
4) If you would like your check mailed to you, please attach a self-addressed stamped envelope to your form. Otherwise your check will be available for pick-up in the treasurer's folder located in the top drawer of the filing cabinet in the workroom at the school.
